APC_INDEX_MISMATCH
What it means
A driver left the kernel in an inconsistent state. Printer, audio and graphics drivers are common causes.
Common causes
- Printer, audio (Realtek) or graphics drivers
- A recent Windows update with a bad driver
How to fix it
- Think about what changed just before it started (new driver, app, update or hardware) and undo that first. System Restore can roll back drivers without touching your files.
- Open the crash dump in WhoCrashed or BlueScreenView: it usually names the driver file that caused it (for example
nvlddmkm.sysis the NVIDIA driver). - Update the graphics, chipset and network drivers from the maker's site (NVIDIA, AMD, Intel or your board vendor), not from a driver-updater tool.
- If it started after a Windows update, uninstall that update (Settings > Windows Update > Update history > Uninstall updates).
